Patient Portal and Healow Policy and Procedures
Sweetwater Medical Associates offers a convenient, secure and confidential way to communicate with their patients through their patient portal. The portal has the latest encryption for secure communication between patients and our office.

- Updates to your demographic information. Always make sure all your information is true, accurate, complete, and updated.

How to Use the Patient Portal for Tele-visit
*Can access via the web patient portal or the Healow app
Step 1 – Call our office to schedule an appointment with your provider and provide your email address to get web-enabled.
Step 2 – You will receive an email with a temporary password. Your username will be your email address.
Step 3 – Create an account using the patient portal (click Patient Portal above page) or by downloading the Healow app from the Apple Store or Google Play.
Step 4 – When you scheduled an appointment you should also have received a reminder to log in for your Tele-visit. ** This is the best way to login to your Tele-visit. Just click on “Join this Telemed Appointment directly”. You may also receive a text message and may join by clicking on the link in the text message.
Step 5 – Once you are logged in to your portal account, you will be directed to the Consent Form and then your Vitals. Please address both items.
Healow ID and Password
To have a tele-visit, I understand that I will create a unique identification (ID) number and password that will be used to access my health information via Healow. This ID and password are unique that identify me in Healow and any inquiries or entries that I make via Healow will be logged with my identity. I understand that it is extremely important that I keep the ID and password confidential and in a safe place. At any time I am able to change my password by going to the password link on the Healow website. It is my responsibility to keep my ID and password safe from any unauthorized access.
Patients who use Healow should be aware they will be notified via e-mail when there is a message or new medical information to be viewed on Healow. Email notifications can be viewed if someone has access to your email account. No private medical information is sent on a notification; however, you must know if you don’t want others to know you are receiving medical information you should consider which email account to provide when setting up Healow. I understand that Healow should never be used to communicate urgent or emergent matters.
By providing my email address and participating in the patient portal/Healow app, I have authorized Sweetwater Medical Associates to notify me with a message sent to the Healow inbox. I will keep my email address up to date and secure at all times.
