Sweetwater Medical Associates is conveniently located on the Houston Methodist Sugar Land Hospital campus in Medical Office Building One. Free parking is available in front of the building and in the parking garage.
Monday – Friday, 8:30 am – 12 pm and 2 pm – 5 pm
Closed for lunch from 12 – 2 pm
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Sweetwater Medical Associates and they will contact us.
If you have an urgent medical need and need to talk to our providers after our regular hours, please call Sweetwater Medical Associates at (281) 494-4900. A physician is on-call 24 hours a day. After hours, your call will be answered by our answering service. One of our doctors will promptly return your call.
When our office is closed we also recommend Next Level Urgent Care for nonemergency medical attention. It is located near Best Buy at the corner of Sweetwater Blvd. and I-69 inside the Physicians Pavilion Urgent Care Building. They are open 7 days a week from 9 am – 9 pm.

At Sweetwater Medical Associates, we accept Medicare and most major insurance carriers and many more. Please contact our office to verify if your insurance plan is accepted. We do not accept Worker's Compensation Injury insurance or Attorney Letters of Protection visits/cases.
If your insurance requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience, we accept cash, checks, Visa, MasterCard, Discover, and American Express. A 3% surcharge will be added to all credit card transactions. The surcharge will not apply to cash and debit cards.
Fees
Credit card dispute reversals will have a charge of $50. NSF checks will have a charge of $50. Missed appointments, no-show appointments, and appointments not cancelled within 24 hours of appointment time will incur a fee from $70 - $100.

Uninsured Cash Prices
An office visit, appointment, or consultation is one that involves a standard medical examination and/or requires any medical decision making to assess, diagnose, and treat a medical condition or issue. Even though these prices are mostly based on time, the nature of your visit also factors into your visit cost.
Well Exams/Physicals are annual comprehensive preventive medicine reevaluation and management, including age and gender appropriate history, counseling, examination, risk factor reduction interventions; and any ordering of appropriate immunization(s), laboratory or diagnostic procedures. This visit is considered a preventive wellness exam and does not cover the discussion of problems, illnesses, conditions, or any other care that would be considered part of a regular visit. Lab fees are also separate from your wellness exam.

If one of our providers suggests a specialist evaluation or diagnostic testing that requires a referral, we will make every effort to provide this for you as soon as possible.
Some referrals require insurance preauthorization and cannot be processed immediately. Insurance guidelines prohibit us from backdating referrals. We require three business days for the processing of routine referrals.
When hospitalization is necessary, we utilize a hospitalist to admit and treat our patients. Hospitalists are physicians with specific expertise in hospital treatment and evaluation. The hospitalist is an extension of our office, and there is two-way communication between physicians. After you are discharged from the hospital we will see you in our office for a follow-up appointment.
To obtain copies of your medical records, please submit your request in writing. Charges may apply for medical records. Persons that are charged for medical records include, but are not limited to patients, attorneys, and insurance companies for determination of new coverage. Prescriptions and refills for established patients of Sweetwater Medical Associates are routinely issued during our regular office hours (see above). You can make your request by using our online prescription refill request form or by calling our office at (281) 494-4900.
No refills are issued during the evenings, weekends, or holidays, as the providers not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
